Hello:

Establishing a constantly functioning "Feedback" loop for
constant performance evaluation purposes is a brilliant suggestion! This is a difficult - but not impossible - process to implement.

Sometimes the most significant factor to prevent its implementation is the normal and understable reluctance of most individuals and organizations to continually be evaluated.

Also the additional workload and data storage concern is a prohibiting factor. Nevertheless, such "Feedback" loops are utilized in private industry and in the military. For example, the airline pilots of certain commercial airlines are constantly being evaluated and tested while performing their routine tasks. The evaluators are not merely their immediate superiors but specially trained performance evaluators who may or may not ever see them again.
