Accountability

What exactly does accountability in the classroom mean? Accountability if there is no student achievement? Does that mean that teachers are fired left and right? How do we verify that the student is able to achieve in that class room? If they don't do homework and outside the class work do we eject them from class? Fine their parents? What are we really talking about here? Achievement is a two part affair at the very least.Can we now finally not pass a student and have them repeat the level as they do in other countries?
